Dear Parent/Guardian of a Day Student:

Every school year, we wait until the end of September before we are able to consider a request for your child. After September 30, any requests will be considered at any time during the school year. Please be advised that the students living in residences are our first priority for the residence staff.

It is important for you to understand that requests for your child to participate in after school activities in residence may be refused, as this will depend upon the appropriate level of supervision available in residences. Alternatively, parent/guardian chaperones may be required for your child in residence during after school activities. In addition, overnight requests will be subject to bed availability and safe, overnight supervision.

Please be aware of the following guidelines when requesting permission for your child to visit Residence:

The Day Student Request for Residence Activity form must be submitted by the parent/guardian to the Residence Supervisor one week in advance of the activity

NOTE: EVERY request for a residence activity requires a permission form- THIS INCLUDES ALL SCHOOL DANCES, unless the teachers are supervising school dances.

Requests will not be accepted where no specific activity is requested, i.e. watching TV, playing computer games, socializing with friends. * Please specify an activity on the form.

Complete the request form (with signatures) and submit it to the Residence Supervisor (Patty Preston or Joyce McFarlane)

No day student will be accepted for a residence activity without a completed Day Student Request for Residence Activity form. If your child arrives at Residence without prior permission, you will be called immediately.

Your child can pick up the form from the Principal’s office or the Residential Services Office. The form will also be available on the school website: www.sjw.reach.net
